Can someone confirm the existence of a revised 2E AD&D "Book of Artifacts" hardcover? I have seen these as softcover only (TSR2138P).

Yeah, they were fairly common.  You can usually pick one up on the cheap on eBay if you are patient (got mine for $3 I think).
