I bought one in an Ebay lot, which was from Titan Games in 2007.  The old IO had one as well.  As for price I could not tell you, nor would I wish to test it on Ebay as Judges Guild products would be one of the last things I ever sell should I need to.

Oh and can I ask where you got that picture?  Don't have one from the old IO least I don't think I do (other then Dungeoneers images he gave me).
