It is from the WOTC page.  I think it is inside their 3.5 edition Player's Handbook II

Yeah, it was a promo, and it was big in 2003 or 2004, IIRC. The bigger image was also broken down into smaller ones for use in selling (or just giving away) bookmarks, keychains, pins, buttons, etc.
