One self-published item called "The Dungeon Master's Index". This is a rather useless index of all the monsters, spells, etc. found in chainmail, the original D&D and its supplements, and the seven issues of The Strategic Review. Dave Arneson was assisted by Dave Meggary (editor) and Heritage Miniatures (graphics).
