Mines of Bloodstone H2...Does it have a fold-out map?

A perspective buyer just asked if the H2 module Mines of Bloodstone came with the map.  As far as I knew there was not a separate fold out map that came with this module.  There are maps on the cover and tri-fold as well as many maps within the module...but a separate fold-out map?  Can someone corfirm this for me?  Thanks

The question might be from Nikosandro over on Dragonsfoot.  There is a map in the middle page of the book that is often removed for use.
