I have here a Dungeon Masters Adventure Log that does not conform to any of the descriptions in the official listings.  I think it's a 4th print if anyone has one to check let me know.  Here is the Acaeum description:

Fourth:  Old-style "Advanced D&D" title centered on top.  Wizard logo.  52 pages total, with 22 two-page adventure logs.  Armor and weapon illustrations in center of book (the two actual copies of this print that we've spotted do not have any illustrations at all; we surmise, however, that the illustration pages were removed by the owners.  Further confirmation needed). Copies have been spotted with and without 3-hole-punching.  Unlike any previous prints, none of the pages are numbered.  The text on the inside cover page is in black ink, as are all the pages.  Cover is in red.  (Thanks to Tony D'Andrea and William Gregory for this info).

The mains points with my 4th print being:

No numbered pages

Armor/Weapon reference is in front and back pages, not attached to the middle.

This leads me to believe the definition of a 4th print in the description is wrong, that the 4th print did NOT have the weapon/armor illustrations in the center, that they were indeed at the front and back and the two examples being incomplete led to a faulty description; anyone else confirm this?

I had to change the title to this thread because I am HOT today!!! :D  I have discovered another print to the 1981 Dungeons & Dragons Player Character Record Sheets (Blue cover Roslof illustration version):

1981:  Player Character Record Sheets.  TSR Face logo.  Blue, full-color cover with Roslof artwork.  16-sheet side-perforated booklet with pale green sheets.  TSR stock # 9037.  Two separate printings have been identified, differing only on their rear covers (thanks to Michael Deaton for this info):

First:  Does not have Piece Code, ISBN, nor Product Code on the bottom rear cover.  Product listing on rear does not have module B3.

Second:  Has Piece Code (101-A-9037), ISBN (0-935696-66-0) and Product Code (394-51546-3TSR0450) on the bottom rear cover.  Product listing contains module B3.

My copy in front of me has NO ISBN code anywhere on the product, it however does have a Piece code and Product code, the Product code is slightly different (first three numbers are "397").  It does list B3 on the back.  Sounds like this is the true 2nd print, and the one above listed as second is actually a third print. Confirmation anyone?

I don't have the answers to either of these questions, but now I have to start picking up those items, too.  Anything with diverse printings draws me in like a magnet for some reason.

Well I can only answer one question.

My 4th print Dungeon Masters Adventure Log is as stated on http://www.acaeum.com/ddindexes/miscpages/dm.html
Except it is 48 pages, has no illustrations (so I assume thats why the 5th is made to correct the lose of illustrations, but they sacrifice the couple pages of logs)
