Can anyone tell me what Dragon the first and last Wormy appeared?  Have they ever been compiled into a book?

Google reveals that Wormy ran from issue 9 issue 132.

Well, I would merely say that he's been located and positively ID'd (by EGG and RJK among others); as to why he quit and took off and what happened to the Wormy collection, I'm not sure that we'll ever know (unless sometake takes a cab ride and gets the chance to interview him in person...).
