I haven't checked the Mail Order catalog but if you have yours handy does it have entries for Unearthed Arcana II and G1-4 Revold of the Giants? I suspect that the colour catalog was put out first and that these may not be in the Mail Order catalog.
We'd be interested in getting a scan of the pre-production mockup cover for this module, which apparently appeared in one or more of the 1986 - 1987 TSR product catalogs.
