I can't find this screen anywhere - not the TSR Archive, not Acaeum, even Google has, thus far, failed me. The copyright is "1979, 1981 TSR Hobbies, Inc. All Rights Reserved."

Inside panels show tables like "Attack Matrix for Monsters" - each of the tables uses the word "matrix".

It's the 2nd screen (quad-fold) which was part of product code 9024.  It was shipped with another screen (bi-fold) folded around it.  So that is the image which gets posted:

Ahhh. Thanks :)  My two screens from that set are 5th and 6th printing, so haven't seen the earlier ones 'in the paper', so to speak ;)
