I've seen that one before, it's one of those latter-versions of the Endless Quest books.

Not Endless Quest, it's from the 1on1 series. This is different. The 1on1 series has two gamebooks inside a slipcase, so that two players can take different sides. The '88 EQ series is just a single paperback for a single player.

It's the German version of Tomb of Horrors. It was released in 1999 for the Fantasy-Spiel-Fest convention. There were 1,000 copies, numbered and signed by Gary Gygax. Here is an example:
