Is anyone familiar with early order blanks? I went through my stash of "extra" and found a couple things, including a June 1979 order form I think I complete. What was the "first" order form like this sent out to folks? I see there is a June and December 1979. There is also a November 1980. Maybe there was one every month? From what year to what year?

I have several of them from April 1978 thru June of 1982.

They seem to come out 2 -3 times a year.
